Occupational Therapist - Hospital Discharge

Location: Islington, London
Rate: -31.37 per hour
Working Pattern: Hybrid
Hours: Monday-Friday, initially 9am-5pm, with some 10am-6pm shifts. Later moving to Monday-Sunday, 1 in 6 rota

Eden Brown Synergy is currently recruiting for a qualified Occupational Therapist to join the Integrated Discharge Service (IDS) within Islington Adult Social Services.

This is an exciting opportunity to join a dynamic and fast-paced Hospital Discharge Service, supporting Islington residents to return home safely while promoting independence, recovery and positive outcomes.



The Role

As an Occupational Therapist, you will play a key role in assessing risk and designing interventions to support residents at different stages of the hospital discharge pathway.

Key responsibilities include:

  • Supporting the Discharge to Assess pathway through referral screening and discharge planning.
  • Participating in the 7-day screening rota, expected to commence after approximately 2 months.
  • Completing post-discharge assessments and providing one-off therapy interventions.
  • Assessing home environments and arranging equipment and minor adaptations.
  • Providing advice on managing daily activities and manual handling.
  • Developing Reablement Plans and Goals using a strengths-based, outcome-focused approach.
  • Working closely with Social Workers, Reablement, Health colleagues and wider multidisciplinary teams.
  • Supporting independence, recovery and admission prevention.

The IDS is co-located with Islington Reablement, providing excellent opportunities for Occupational Therapists to shape Reablement Plans from the outset of a resident's care journey.



What We're Looking For

  • Qualified Occupational Therapist.
  • Relevant professional registration.
  • Experience in adult social care, hospital discharge, community or reablement settings is desirable.
  • Strong assessment, risk management and clinical decision-making skills.
  • A person-centred and strengths-based approach.
  • Ability to work effectively within a multidisciplinary team.
  • Excellent communication and organisational skills.
